Airworthiness Directives; Airbus Airplanes

We propose to supersede Airworthiness Directive (AD) 90-11-05 for certain Airbus Model A300 series airplanes and Model A300 B4-600 series airplanes. AD 90-11-05 currently requires repetitive detailed inspections for cracking in the aft hinge brackets of the outer shroud box that is located in the outer wing box, and related investigative and corrective actions if necessary. Since we issued AD 90-11-05, we have determined that a change to certain compliance times is needed. This proposed AD would continue to require doing repetitive detailed inspections for cracking in the hinge brackets of the forward and aft outer shroud boxes that are located in the outer wing box, and related investigative and corrective actions if necessary; and would add airplanes to the applicability. We are proposing this AD to detect and correct cracking of the aft hinge brackets of the outer shroud box; such cracking could affect the structural integrity of the airplane.
Establishment of Class E Airspace, Neah Bay, WA

This action establishes Class E airspace at U. S. Coast Guard Station Neah Bay Heliport, Neah Bay, WA, to accommodate a new Standard Instrument Approach Procedure developed at the heliport. Controlled airspace is necessary for the safety and management of Instrument Flight Rules (IFR) operations at the heliport.
Proposed Amendment of Class E Airspace; Deer Lodge, MT

This action proposes to modify Class E airspace extending upward from 700 feet above the surface at Deer Lodge-City-County Airport, Deer Lodge, MT. After a review, the FAA found it necessary to amend the airspace area for the safety and management of standard instrument approach procedures for Instrument Flight Rules (IFR) operations at the airport.
Airworthiness Directives; The Boeing Company Airplanes

We propose to adopt a new airworthiness directive (AD) for certain The Boeing Company Model 787-8 and 787-9 airplanes equipped with General Electric engines. This proposed AD was prompted by reports of cracking in barrel nuts on a forward engine mount of Model 747-8 airplanes, which shares a similar design to the forward engine mount of Model 787-8 and 787-9 airplanes. This proposed AD would require, for certain airplanes, replacement of the four barrel nuts of the forward engine mount on each engine. For certain other airplanes, this proposed AD would require an inspection to determine if any forward engine mount barrel nut having a certain part number is installed, and related investigative and corrective actions if necessary. We are proposing this AD to detect and correct cracking of the forward engine mount barrel nuts; such cracking could result in reduced load capacity of the forward engine mount and could result in separation of an engine from the airplane, and consequent loss of control of the airplane.
Airworthiness Directives; Airbus Airplanes

We propose to supersede Airworthiness Directive (AD) 2014-03- 14, for all Airbus Model A330-200 and -300 series airplanes, and Model A340-200, -300, -500, and -600 series airplanes. AD 2014-03-14 currently requires removing bulb-type maintenance lights; installing a drain mast on certain airplanes; and installing muffs on connecting bleed elements on certain airplanes. Since we issued AD 2014-03-14, we have determined that additional actions are necessary to address the identified unsafe condition for certain airplanes on which muffs are installed. For certain Model A340-200 and -300 series airplanes, this proposed AD would also require replacing certain insulation sleeves with new insulation sleeves. We are proposing this AD to prevent ignition sources inside fuel tanks, which, in combination with flammable fuel vapors, could result in fuel tank explosions and consequent loss of the airplane.

These special conditions are issued for the Cirrus Aircraft Corporation Model SF50 airplane. This airplane will have a novel or unusual design feature(s) associated with installation of an Auto Throttle System. The applicable airworthiness regulations do not contain adequate or appropriate safety standards for this design feature. These special conditions contain the additional safety standards that the Administrator considers necessary to establish a level of safety equivalent to that established by the existing airworthiness standards.

Airworthiness Directives; Agusta S.p.A. Helicopters

We are adopting a new airworthiness directive (AD) for Agusta S.p.A. (Agusta) Model A109A and A109A II helicopters. This AD requires inspecting the slider assembly pitch control (slider) for play and replacing the slider if the play exceeds certain limits. This AD is prompted by a report of excessive slider play and wear that was detected during a scheduled inspection of a Model A109A II helicopter. These actions are intended to detect and prevent excessive wear and play on a slider, which could lead to loss of tail rotor pitch control and consequently loss of helicopter control.

We propose to adopt a new airworthiness directive (AD) for certain Pratt & Whitney Division (PW) PW4000-94 inch and PW4000-100 inch model turbofan engines. This proposed AD was prompted by a report of a crack find in the high-pressure compressor (HPC) disk. This proposed AD would require performing an ultrasonic inspection (USI) or an eddy current inspection (ECI) of the HPC 10th stage disk. We are proposing this AD to prevent failure of the HPC 10th stage disk, an uncontained disk release, damage to the engine, and damage to the airplane.
Airworthiness Directives; Rolls-Royce plc Turbofan Engines

We propose to adopt a new airworthiness directive (AD) for certain Rolls-Royce plc (RR) RB211-22B and RB211-524 turbofan engines with low-pressure turbine (LPT) support roller bearing, part number (P/ N) LK30313 or P/N UL29651, installed. This proposed AD was prompted by a report of a breach of the turbine casing and release of engine debris. This proposed AD would require removal of certain LPT support roller bearings installed in RR RB211-22B and RB211-524 engines. We are proposing this AD to prevent failure of the LPT support roller bearing, loss of radial position following LPT blade failure, uncontained part release, damage to the engine, and damage to the airplane.
Airworthiness Directives; Piper Aircraft, Inc. Airplanes

We propose to supersede Airworthiness Directive (AD) 96-12-12, which applies to certain Piper Aircraft, Inc. Models PA-31, PA-31-300, PA-31-325, and PA-31-350 airplanes. AD 96-12-12 currently requires a one-time inspection of the bulkhead assembly at fuselage station (FS) 317.75 for cracks and the installation of one of two reinforcement kits determined by whether cracks were found during the inspection. Since we issued AD 96-12-12, bulkhead cracks were found on airplanes that had complied with AD 96-12-12 and on additional airplanes not affected by AD 96-12-12. This proposed AD would require repetitive inspections of the bulkhead assembly at FS 317.75 for cracks, repair of cracks as necessary, and the installation of a reinforcement modification. We are proposing this AD to correct the unsafe condition on these products.
Airworthiness Directives; Piper Aircraft, Inc. Airplanes

We are adopting a new airworthiness directive (AD) for certain Piper Aircraft, Inc. Models PA-23-250, PA-24-250, PA-24-260, PA-24-400, PA-30, PA-31, PA-31-300, PA-31P, PA-39, and PA-E23-250 airplanes. This AD was prompted by an accident caused by fuel starvation where the shape of the wing fuel tanks and fuel below a certain level in that tank may have allowed the fuel to move away from the tank outlet during certain maneuvers. This AD requires installing a fuel system management placard on the airplane instrument panel and adding text to the Limitations Section of the pilot's operating handbook (POH)/airplane flight manual (AFM). We are issuing this AD to correct the unsafe condition on these products.
Office of Commercial Space Transportation; Notice of Availability and Request for Comment on the Second Draft Environmental Assessment (EA) for the Kodiak Launch Complex Launch Pad 3, Kodiak Island, Alaska

In accordance with the National Environmental Policy Act of 1969, as amended (NEPA; 42 United States Code 4321 et seq.), Council on Environmental Quality NEPA implementing regulations (40 Code of Federal Regulations parts 1500 to 1508), and FAA Order 1050.1E, Change 1, Environmental Impacts: Policies and Procedures, the FAA is announcing the availability of and requesting comments on the Second Draft Environmental Assessment for the Kodiak Launch Complex Launch Pad 3 (Second Draft EA). On September 15, 2014, the FAA issued a Draft EA for public review and comment and received 54 written comments and 26 oral comments over the public comment and review period that was extended to November 1, 2014. After taking into consideration the nature of public comments received on the Draft EA, the FAA is providing the public with an opportunity to review and comment on updates and clarification information that have since been added to the EA in response to public comments. The FAA is issuing an updated version of the Draft EA for a second public review and comment period.

We propose to adopt a new airworthiness directive (AD) for all General Electric Company (GE) CF6-80C2 and CF6-80E1 turbofan engines. This proposed AD was prompted by reports of a burn-through of the accessory heat shield during an accessory compartment fire leading to an engine fire. This proposed AD would require replacing the accessory heat shield assembly. We are proposing this AD to prevent high- temperature gas ingestion into the accessory compartment, engine fire, and damage to the airplane.

Airworthiness Directives; Gulfstream Aerospace Corporation Airplanes

We are adopting a new airworthiness directive (AD) for certain Gulfstream Aerospace Corporation Model GVI airplanes. This AD requires repetitive breakaway torque checks and torqueing of the brake inlet self-sealing couplings. This AD also requires revising the airplane flight manual to include procedures to follow in the event of certain display indications. This AD was prompted by reports of the self- sealing couplings on the brake inlet fitting that have been found backed out of the fully seated position. We are issuing this AD to detect and correct inadequate torque on the self-sealing coupling. This condition could result in an unannounced total loss of braking capability on one or multiple brakes, which could result in a runway overrun or asymmetrical braking that can lead to a lateral runway excursion.

Airworthiness Directives; Airbus Helicopters

We are publishing a new airworthiness directive (AD) for Airbus Helicopters Model AS350B3 helicopters. This AD was sent previously to all known U.S. owners and operators of these helicopters and supersedes Emergency AD 2015-22-52, dated October 28, 2015. This AD requires revising the rotorcraft flight manual (RFM) to stop performing the yaw load compensator check during preflight procedures and instead perform it during post-flight procedures after rotor shut-down. This AD also requires the yaw servo hydraulic switch to be in the ``ON'' position before taking off. This AD is prompted by two accidents and one incident of Airbus Helicopters Model AS350B3 helicopters. From preliminary investigations, loss of tail rotor (T/R) control during takeoff was evident in each event. These actions are intended to prevent takeoff without hydraulic pressure in the T/R hydraulic system, loss of T/R flight control, and subsequent loss of control of the helicopter.
