Rule R398-5 - Birth Defects and Critical Congenital Heart Disease Reporting
Section R398-5-5 - Critical Congenital Heart Disease (CCHD) Screening Reporting

The institution shall report or cause to report the CCHD Screening results to the department within 40 days of discharge, this includes the following:

(1) the newborn's last name;

(2) the newborn's first name;

(3) the newborn's date of birth;

(4) the newborn's gender;

(5) the newborn's gestational age;

(6) the newborn's birth weight;

(7) the newborn's medical record number;

(8) the newborn's newborn screening kit number;

(9) the newborn's delivery institution;

(10) the newborn's discharge unit, if applicable;

(11) the newborn's CCHD Screening result for each attempt, this includes:

(a) date;

(b) time;

(c) probe location;

(d) SpO2 result; and

(e) outcome of attempt;

(12) the newborn's first echocardiogram, if indicated:

(a) date; and

(b) time;

(13) the mother's last name;

(14) the mother's first name;

(15) the mother's maiden name;

(16) the mother's date of birth; and

(17) the mother's medical record number.
