Section 410.4 - Application for registration

Applications for registration shall be made in such form as may be prescribed by the superintendent and may be obtained by written request to the Banking Department, at the address set forth in section 1.1 of Supervisory Policy G 1 of this Title.

(a) Fee. A completed application must be accompanied by payment of the fee specified in section 1.2 of Supervisory Policy G 1 of this Title.

(b) Application form. The application for registration as a mortgage broker shall be in writing, subscribed by the applicant under penalty of perjury, and shall include the information and documents referred to in Supervisory Procedure MB 102 of this Title as well as any other information and documents as the superintendent may require.

(c) Fingerprints. The applicant shall provide a set of fingerprints in such form as shall be accepted for processing by the New York State Criminal Justice System.
