(a) Any person may request the board to commence a proceeding for the purpose of adopting, amending, or repealing a rule by filing a written petition that contains:
(1) The text of the proposed rule or a
statement of the particular results intended by the petitioner to flow from the
implementation of the proposed rule;
(2) An identification of the particular rule
sought to be amended or repealed;
(3) Any data or argument the petitioner
believes would be useful to the board in deciding whether to commence a
rulemaking proceeding; and
(4)
Name, address, signature of petitioner and date.
