A licensee terminating deferred deposit advance operations must
(1) provide electronic notice to the department through the registry of a decision to cease operations or the cessation of operations not later than seven days after that decision or cessation; and
(2) submit a completed request for surrender; and
(3) submit reports as required under 3 A AC 11.050 through the date of tennination of operations.
