The Federal Emergency Management Agency (FEMA) has submitted the following information collection to the Office of Management and Budget (OMB) for review and clearance in accordance with the requirements of the Paperwork Reduction Act of 1995. Title: Federal Assistance for Offsite Radiological Emergency. OMB Number: 1660-0024. Abstract: In accordance with Executive Order 12657, FEMA \1\ will need certain information from the licensee in order to form a decision as to whether or not a condition of ``decline or fail'' exists on the part of State or local governments (44 CFR 352.3-4). This information will be collected by the appropriate FEMA Regional Office or Headquarters. Also in accordance with the Executive Order, when a licensee requests Federal facilities or resources, FEMA will need information from the NRC as to whether the licensee has made maximum use of its resources and the extent to which the licensee has complied with 10 CFR 50.47 (c)(1) and 44 CFR 352.5. This information will be collected by the NRC and will be provided to FEMA through consultation between the two agencies.

Abstract: The application for exemption is made to the Federal Insurance Administration by the Governor or other duly authorized official of the State accompanied by sufficient supporting documentation which certifies that the plan of self-insurance upon which the application for exemption is based meets or exceeds the standards set forth in 44 CFR 75.11. Upon determining that the State's plan of self-insurance equals or exceeds the standards, the Administrator then certifies that the State is exempt from the requirements for the purchase of flood insurance for State-owned structures and their contents. The application form will enable FEMA to continue to rapidly process new community applications and to thereby more quickly provide flood insurance protection to the residents of the communities. Participation in the NFIP is mandatory in order for flood related Presidentially-declared communities to receive Federal disaster assistance. Affected Public: State, local or Tribal Governments. This notice amends the notice of a major disaster declaration for the State of Indiana (FEMA-1612-DR), dated November 8, 2005, and related determinations.
Suspension of Community Eligibility
This rule identifies communities, where the sale of flood insurance has been authorized under the National Flood Insurance Program (NFIP), that are scheduled for suspension on the effective dates listed within this rule because of noncompliance with the floodplain management requirements of the program. If FEMA receives documentation that the community has adopted the required floodplain management measures prior to the effective suspension date given in this rule, the suspension will not occur and a notice of this will be provided by publication in the Federal Register on a subsequent date.
Changes in Flood Elevation Determinations
This interim rule lists communities where modification of the Base (1% annual-chance) Flood Elevations (BFEs) is appropriate because of new scientific or technical data. New flood insurance premium rates will be calculated from the modified BFEs for new buildings and their contents.
Proposed Flood Elevation Determinations
Technical information or comments are requested on the proposed Base (1% annual-chance) Flood Elevations (BFEs) and proposed BFE modifications for the communities listed below. The BFEs and modified BFEs are the basis for the floodplain management measures that the community is required either to adopt or to show evidence of being already in effect in order to qualify or remain qualified for participation in the National Flood Insurance Program (NFIP).

This information is required to determine the feasibility of the site for installation of the housing unit and ensures written permission of the property owner is obtained to allow the housing unit on to the property to include ingress and egress permission. Affected Public: Individuals or Households. Number of Respondents: 1,000 respondents. Estimated Time per Respondent: 20 minutes or .33 hours (10 minutes for FF 90-1 and 10 minutes for FF 90-31). The information is needed to improve instruction and content. Demographic data are needed to identify differentials in course impact. Title: Revisions to National Flood Insurance Program Maps: Application Forms and Instructions for (C) LOMAs and (C) LOMR-Fs. OMB Number: 1660-0015. Abstract: The certification forms (referred to as MT-1 series forms) are designed to assist requesters in gathering information that FEMA needs to determine whether a certain property is likely to be flooded during a flood event that has a one-percent annual chance of being equaled or exceeded in any given year (base flood). FEMA Form 81-87, Property Information, describes the location of the property, what is being requested, and what data are required to support the request. FEMA Form 81-87A, Elevation Information, indicates what the Base (one-percent annual chance) Flood Elevation (BFE) for the property is, how the BFE was determined, the lowest ground elevation on the property, and/or the elevation of the lowest adjacent grade to any structures on the property. This information is required in order for FEMA to determine if the property that the requester would like removed from the SFHA is at or above the BFE. FEMA Form 81-87B, Community Acknowledgment, requires that a community official certify that the request complies with minimum floodplain management criteria specified in 44 CFR 60.3, as per NFIP regulations 44 CFR 65.5(a)(4). Collection of Information: Title: Effectiveness of a Community's Implementation of the NFIP Community Assistance Program CAC and CAV Reports. OMB Number: 1660-0023. Abstract: This collection provides basic information about floodplain management program and contact information pertaining to NFIP participating communities. The information is used by FEMA and NFIP State Coordinating Agencies to identify problems with communities' floodplain management and determine assistance. The two key methods FEMA uses in determining community assistance needs are through the CAC and CAV, which serve to provide a systematic means of monitoring community NFIP compliance. Through the CAC and CAV, FEMA can also determine to what extent communities are achieving the flood loss reduction objectives of the NFIP. By providing assistance to communities, the CAC and CAV also serve to enhance FEMA's goals of reducing future flood losses, thereby achieving the cost-containment objective of the NFIP. Affected Public: State, Local, Tribal, and Federal Governments. Number of Respondents: 3,000. Estimated Time per Respondent: Two hours per respondent to complete the Community Visit Report (CAV), FF 81-68; and 1 hour per respondent to complete the Community Contact Report (CAC), FF 81-69. The 9/11 Heroes Stamp Act of 2001 directed the United States Postal Service to issue a semipostal stamp and distribute the proceeds through the Federal Emergency Management Agency to the families of emergency relief personnel killed or permanently disabled while serving in the line of duty in connection with the terrorist attacks against the United States on September 11, 2001. This notice announces the application period for the Assistance Program Under the 9/11 Heroes Stamp Act of 2001.
Fee Schedule for Processing Requests for Map Changes, for Flood Insurance Study Backup Data, and for National Flood Insurance Program Map and Insurance Products; Correction
The Federal Emergency Management Agency published a document in the Federal Register of September 19, 2005, concerning the revised fee schedules for processing certain types of requests for changes to NFIP maps, requests for FIS technical and administrative support data, and requests for particular NFIP map and insurance products. The document contained the incorrect fee associated for processing requests for Conditional Map Revisions based on new hydrology, bridge, culvert, channel, or combination thereof.
