Background: This collection of information is required to administer the benefit payment provisions of the Black Lung Act for survivors of deceased miners. amended, the Department of Labor issued a Certification of Eligibility 
to Apply for Worker Adjustment Assistance and Alternative Trade 
Adjustment Assistance on January 15, 2009, applicable to workers of
