Abstract: The data collection
instruments being submitted are the
annual performance reports for State
Independent Living Services (SILS) and
Centers for Independent Living (CIL)
programs. These are known as the 704
Report Part I and the 704 Report Part II,
respectively. These reports are required
by sections 704(m)(4)(D), 706(d),
721(b)(3) and 725(c) of the
Rehabilitation Act of 1973, as amended
(the Act) and the corresponding
regulations in 34 CFR parts 364, 365,
and 366. Approval of grantees’ annual
performance reports (704 Report) is a
prerequisite for RSA’s approval of the
annual SILS grant awards (part B funds)
and CILs continuation grant awards
(part C funds).
