Section 2-13 - Reinstatement After Disciplinary Action

(a) A licensee whose license has been suspended may petition the Board for reinstatement of the license thirty (30) days before the end of the suspension period.
(i) A petition for reinstatement shall be
accompanied by at least two (2) verified recommendations from licensed Nursing
Home Administrators and by at least two (2) recommendations from individuals
each having personal knowledge of the activities of the licensee since the
disciplinary action was imposed.
(ii) A petition for reinstatement shall be
considered at the next Board meeting.
(iii) Any petition for reinstatement of a
license shall be acted upon by the Board within one hundred and eighty (180)
days after the filing of a proper petition and the accompanying
documents.
(iv) A licensee whose
license is reinstated shall pay the renewal fee for the year in which the
license is reinstated.
