Clients of this program must:
(a) Be at least 18 years of age;
(b) Be determined through an ongoing assessment to be facing the possibility of premature institutionalization;
(c) Meet but not exceed the Level of Care requirements as established by the Division's evaluation, and;
(d) Reside within the State of Wyoming.
