(a) The following is a list of individuals and facilities which have an independent duty to report the occurrence of sexually transmitted disease:
(i) A physician or other health care provider
diagnosing or treating a case of sexually transmitted disease;
(ii) The administrator of a health care
facility or penal institution in which there is a case of a sexually
transmitted disease;
(iii) The
administrator or operator of a laboratory performing a positive test for
sexually transmitted disease.
