Chapter 1 - MEDICAL MALPRACTICE INSURANCE ASSISTANCE PROGRAM
Section 1-7 - Application Form

(a) To be eligible to participate in this program a physician or contracting entity shall submit an application to the Department upon application forms approved by the Department;

(b) The Application shall include forms, eligibility criteria, rights and responsibilities regarding receiving assistance;

(c) An application shall be provided upon request;

(d) A separate application shall be required for each applicant;

(e) The application may be mailed or hand-delivered to the Department;

(f) Applications must be post-marked or date-stamped by the Department on or before March 30, 2007;

(g)The application must be complete prior to consideration by the Department
