(a) Durable medical equipment provided by a hospital to a recipient for home use is covered pursuant to the rules and policies of the Department if the hospital is enrolled as a Medicaid provider of medical supplies.

(b) Laboratory services are covered if based on specific, written orders of a physician;

(i) Specimen collection fees are reimbursable only to the provider that extracts the specimen;

(ii) Only one collection fee is allowed for each type of specimen for each recipient encounter, regardless of the number of specimens extracted;

(iii) Clinical laboratory services routinely performed by non-physicians are not entitled to a professional component;

(c) Medical-surgical supplies are covered if prescribed by a physician and to the extent required for the reduction of physical disability and to restore the recipient's functional level.

(d) Orthotics and prosthetics are covered if provided to an inpatient or to an outpatient if part of outpatient hospital services.

(e) Pharmaceuticals. Medicaid covers pharmaceuticals provided by a hospital as follows:

(i) Inpatient pharmaceuticals are reimbursed pursuant to Chapter 30.

(ii) Outpatient pharmaceuticals are reimbursed pursuant to Chapter 33.

(f) Radiology services. Medicaid covers radiology services provided by a hospital.
