Chapter 3 - CERTIFICATION REQUIREMENTS AND APPLICATION PROCEDURE

Applicants for certification must submit a complete and legible application form and provide the following items;

(a) A non-refundable fee of $100.00

(b) Acceptable proof of U.S. citizenship or legal presence in the U.S.

(c) A certificate of completion of an approved animal euthanasia training and testing program as defined in Section 2, or

(i) If the training was provided by a veterinarian licensed in the State of Wyoming, an Affidavit of Training on the form provided by the Board.

(d) A criminal history background check along with a fingerprint card completed by a law enforcement agency. The criminal history background check shall include copies of all law enforcement agency records relating to the applicant for a period of fifteen (15) years prior to the application date. Any fees incurred with obtaining this information shall be paid by the applicant.

(e) Official verification of certification from jurisdictions where the applicant holds or has held certification as an animal euthanasia technician.
