Chapter 3 - CERTIFICATION REQUIREMENTS AND APPLICATION PROCEDURE
Section 3-3 - Application Procedures

(a) All documents submitted in support of the application must contain an original signature and be submitted directly to the Board office from the respondent and not forwarded through the applicant.
(b) The Board will not accept faxed or photocopied documents.
(c) The applicant must document completion of all requirements within six (6) months of the application date or the application will be deemed abandoned and the file closed.
(d) Each applicant must file with the Board their current home and professional mailing addresses and telephone numbers and must report to the Board in writing any change of addresses or telephone numbers, giving both old and new addresses and telephone numbers.
