Section NR 664.1036 - Reporting requirements

(1) Owners and operators subject to this subchapter shall submit a semiannual report to the department by dates specified by the department. The report shall include all of the following information:
(a) The EPA
identification number, name and address of the facility.
(b) For each month during the semiannual
reporting period, dates when the control device exceeded or operated outside of
the design specifications as defined in s.
NR 664.1035(3)
(d) and as indicated by the control device
monitoring required by s.
NR 664.1033(6) and the exceedances were
not corrected within 24 hours, or that a flare operated with visible emissions
as defined in s.
NR 664.1033(4) and as determined by
Method 22 in appendix A of 40 CFR part 60, incorporated by reference in s.
NR 660.11, monitoring, the duration and cause of each
exceedance or visible emissions and any corrective measures taken.
(2) If, during the semiannual reporting period, the control device does not exceed or operate outside of the design specifications as defined in s. NR 664.1035(3) (d) for more than 24 hours or a flare does not operate with visible emissions as defined in s. NR 664.1033(4), a report is not required.
