Title 126 - PROCEDURAL RULE BOARD OF EDUCATION
Series 126-087 - Uniform Health Record Policy (4322)
Section 126-87-2 - Applicability

Effective at the beginning of the 1981-82 school year, the uniform health record card shall be adopted statewide for the following categories of pupils:

1. All pupils enrolling in kindergarten and prekindergarten programs.

2. First graders who did not attend kindergarten.

3. All transfers from other schools.

Counties desiring to use another health card must request an exception from the State Board of Education. However, the State Board will not approve a local health record that does not contain the data required by the state adopted uniform health record.
