A. The commissioner notifies the requester, or other adult if applicable, and the sponsor of his approval or denial in writing by certified mail.
B. Any approved waiver is for a specific person and a specific facility and must include:
1. Name of individual;
2. Name of facility;
3. Effective dates;
4. Terms, conditions, and stipulations, if
any;
5. Criminal conviction for
which the waiver was granted;
6.
Date of criminal conviction;
7.
Relevant court and location;
8.
Sentence served; and
9. Signature
of commissioner, or designee, and date.
