Section R708-48-10 - Procedure to Apply for and Renew an Installer License

(1) An applicant for a new or renewal of an existing installer license, shall mail a complete installer application packet to the division at Box 144501 Salt Lake City, Utah 84114.
(2) The packet under Subsection (1) shall include:
(a) an installer application form
provided by the division, which has been signed by the applicant and any other
required parties;
(b) an
application fee or renewal fee, which shall be made payable to the
department;
(c) a signed agreement
verifying that the applicant has read and understands each of the laws and
rules that are applicable to Title 53, Chapter 3, Part 10, Ignition Interlock
System Program Act;
(d) one FBI
applicant fingerprint card with the applicant's legible fingerprints with
initial application and upon renewal; and
(e) a check or money order made payable to
the Utah Bureau of Criminal Identification to cover the cost of a background
check.
(3) The applicant shall include original documents in the packet unless the division requests photocopies.
(4) An installer may not transfer their license.
(5) The installer may apply for a duplicate installer license by submitting a written request and a duplicate license fee to the division.
(6) The division may not process an application if any requirement is not met.
