Section R654-1-5 - Application for Permit to Survey

(a) A person who wishes to obtain a permit to survey shall obtain and complete an application form and submit the form and all other information required by the form to the Public Lands Policy Coordinating Office.
(b) Other required information may include:
(i) Projects initiated under previous permits
issued by the state that remain incomplete as of the date of
application.
(ii) The applicant's
employer or the name of the applicant's business, if self-employed.
(iii) A copy of an agreement to curate with
an authorized curation facility in the name of the applicant or the applicant's
employer.
