1. Approvals or orders authorizing injection wells shall be valid for the life of the well, unless revoked by the board for just cause, after notice and hearing.
2. An approval may be administratively amended if:
2.1. There is a substantial
change of conditions in the injection well operation.
2.2. There are substantial changes to the
information originally furnished.
2.3. Information as to the permitted
operation indicates that an USDW is no longer being protected.
