Section R436-2-3 - Foundling Certificates

(1) The Office shall include the following information on the foundling certificate form provided by the Office:

(a) Information from the hospital that receives an infant of unknown parentage found in the state, including:
(i) the date and address of the place of finding, which will be used as the place of birth;

(ii) the sex, approximate live birth date, and race of the infant if known;

(iii) the name and address of the person or institution submitting the report, and:

(iv) other medical information, if available.

(b) The name for the infant will be obtained from the Division of Child and Family Services .

(2) Information submitted under this section shall constitute the basis for the report of live birth for the infant.
