Section R432-270-14 - Service Plan

(1) The licensee shall ensure that each resident has an individualized service plan that is consistent with the resident's unique cognitive, medical, physical, and social needs, and is developed within seven calendar days of the day the facility admits the resident. The licensee shall ensure the service plan is periodically revised as needed.

(2) The licensee shall ensure the resident assessment is used to develop, review, and revise the service plan for each resident.

(3) The licensee shall ensure that the service plan includes a written description of the following:

(a) the services to be provided;

(b) who will provide the services, including the resident's significant others who may participate in the delivery of services;

(c) how the services are provided;

(d) the frequency of services; and

(e) changes in services and reasons for those changes.
