Rule R414-508 - Requirements for Transfer of Bed Licenses
Section R414-508-3 - Bed License Transfer Requirements for the Transferor

(1) A nursing care facility program shall meet the requirements of Rule R414-27 to fulfill the transfer requirements found in Subsection 26B-3-313(2).

(a)
Pursuant to Subsection
26B-3-313(2), a
nursing care facility program shall demonstrate its intent to transfer bed
licenses by providing written notice to the Division of Integrated Healthcare
(DIH) in accordance with timing specified in Section 26B-3-313.
(b) To request the transfer under Subsection
(a), the transferring nursing care facility program or entity shall use the
form that provides notice of a Medicaid bed transfer.
(3)
(a) The
nursing care facility program shall include necessary information on the notice
form.
(b) If the notice form or
supporting documentation is deficient, DIH shall return the incomplete notice
to the requestor.
(4) The notice date is the date the completed notice form is emailed to DIH at qii@utah.gov.
