(1) This rule implements the immunization requirements of Title 53G, Chapter 9, Part 3. It establishes minimum immunization requirements for attendance at a public, private, elementary, or secondary school through grade 12, nursery school or Kindergarten, licensed day care center, child care facility, family home care, or Head Start program in this state. It establishes:
(a) required doses and frequency of vaccine
administration;
(b) reporting of
statistical data; and
(c) time
periods for conditional enrollment.
(2) This rule is required by Section 53G-9-305 and authorized by Section 53G-9-308.
