A. Prior to taking an examination, an applicant must file an application of intention with the director using an approved form, accompanied by:
1. evidence of qualifications for
certification in accordance with the provisions of Section
R317-10-11;
2. the appropriate fee; and
3. documentation that requirements for
certification of citizenship or alien residency are met.
B. Approved forms are available on the internet at www.waterquality.utah.gov.
