Section R162-2g-304f - Final Application for Licensure or Certification

(1) Within 90 days after successfully completing the exam for licensure or certification, the applicant shall return to the division:
(a) a report from the
testing service indicating successful completion of the exam within 24 months
of the date on which the applicant obtains authorization to sit for the
exam;
(b) an application form as
required by the division and including:
(i)
the applicant's business, home, and e-mail addresses;
(ii) the name and business address of any
appraisal entity or government agency with which the applicant is affiliated;
and
(iii) if the applicant is
applying for certification, the fee for the federal registry.
(2)
(a) A post office box without a street
address is unacceptable as a business or home address.
(b) An applicant may designate any address to
be used as a mailing address.
