Section 21.2008 - Information Provided by an Insurer

(a) Upon request, an insurer shall provide to a state agency providing medical assistance, or to a child support agency enforcing medical support, information necessary to facilitate reimbursement of medical services provided to or paid on behalf of a child.

(b) An insurer providing coverage to a child through a parent of the child shall provide information and documents within 31 days after coverage has been made permanent or upon written request, whichever occurs first, to each custodial parent or a child over 18 years of age as may be necessary for the child to obtain benefits through that coverage, including:

(1) the name of the insurer;

(2) the number of the policy;

(3) a copy of the policy and schedule of benefits;

(4) a health insurance membership card;

(5) claim forms; and

(6) any other information or document necessary to submit a claim in accordance with the insurer's policies and procedures.
