Section 747.1131 - What additional documentation must I submit to show I am qualified to be a primary caregiver of a licensed child-care home?

(a) In addition to showing that you meet the minimum qualifications for a primary caregiver, you must submit the following to Licensing staff:
(1) A completed Licensing Personal History
Statement form specifying your education and experience;
(2) An original and current Licensing
Child-Care Director's Certificate form, an original college transcript, or
original training certificates which verify the educational requirements.
Original letters may be substituted for training certificates, provided they
include the same information as specified in §
747.1327 of this subchapter
(relating to What documentation must I provide to Licensing to verify that
caregivers have met training requirements ?); and
(3) Complete dates, names, addresses, and
telephone numbers which support the required experience.
(b) You must submit the information to us as a part of a new application for a permit.
