Section 559.39 - Voluntary Closure

(a) A provider must notify HHSC in writing at least five days before permanent closure of the operation.
(b) The provider must include in the written notice:
(1) the date of
permanent closure;
(2) the reason
for closing;
(3) the location of
individual records, both active and inactive; and
(4) the name and address of the individual
records custodian.
