Section 356.1802 - Personnel Files

A center must maintain a personnel file for each employee. Each file must include at least the following information:

(1) employment application or resume;

(2) job descriptions;

(3) signed acknowledgment of confidentiality agreement;

(4) signed acknowledgment of receipt of personnel policies and procedures handbook;

(5) performance evaluations for every year of employment in accordance with the center's personnel policies;

(6) documentation of orientation, initial training, and employee development;

(7) any status or classification change;

(8) all disciplinary actions, if any; and

(9) letters of praise or criticism, if any.
