Section 356.402 - Personnel Files

A center must maintain a personnel file for each employee. Each file must include at least the following information:
(1) Employment application or resume;
(2) Job descriptions;
(3) Signed acknowledgment of confidentiality agreement;
(4) Signed acknowledgment of receipt of personnel policies and procedures handbook;
(5) Performance evaluations for every year of employment in accordance with the center's personnel policies;
(6) Documentation of orientation, initial training, and employee development;
(7) Any status or classification change;
(8) All disciplinary actions, if any; and
(9) Letters of praise or criticism, if any.
