Section 279.101 - Termination

(a) If DADS terminates ERS, a provider may be paid for the last month of service, regardless of how many days of service were provided that month, if:
(1) the provider has
already conducted a system check that month before the termination of
services;
(2) the provider
conducted a system check on the day it picked up the equipment; or
(3) the provider could not complete a system
check because:
(A) the individual's telephone
was disconnected;
(B) the
individual damaged the equipment;
(C) the equipment was picked up at a location
other than the individual's home; or
(D) the individual changed his telephone
number or address without allowing the provider to remove the equipment from
the individual's home.
(b) The provider must:
(1) document the results of the final system
check; or
(2) document the reason
the provider was unable to complete a system check.
