Section 261.208 - Provider Agreement

(a) DADS enters into a provider agreement only with a provider applicant that has received licensure under THSC, Chapter 252, if applicable, and certification by DADS in accordance with § 261.207 of this chapter (relating to Certification and Licensure).
(b) The effective date of a provider agreement is the effective date of certification by DADS.
(c) A provider agreement remains in effect until it is terminated.
