Section 229.426 - Report of Changes

(a) Change in the content of a license application. The license holder must notify the department in writing within 10 days of any change which would render the information contained in the application for the license, reported pursuant to § 229.425 of this subchapter (relating to Licensing Procedures), no longer accurate. Failure to inform the department no later than 10 days of a change in the information required in the application for a license may result in an enforcement action, including suspension or revocation of the license.
(b) Change in location of place of business. The license holder must notify the department at least 30 days in advance of an intended change of address of the licensed place of business. The notice must include the address of the new location and the name of the individual in charge of the business at the new location. Within 10 days of beginning operations at the new location, the license holder must notify the department in writing to confirm the move, and provide verification or correction of the information provided on the notice of intent. The notice and confirmation required by this subchapter will be deemed adequate if the license holder submits the notices by certified mail, return receipt requested, to the Texas Department of State Health Services, 1100 West 49th Street, Austin, Texas, 78756, or electronically through texas.gov.
