Section 601.10 - License Issuance and License Holder Requirements

(a) The board may issue a license to an eligible applicant if the applicant meets all license requirements under the Act and this chapter.
(b) A license certificate is the property of the board and must be surrendered on demand.
(c) A licensee shall display or make available the license in an appropriate manner.
(d) A licensee shall notify the board of any change in name, preferred mailing address, or place(s) of business or employment within 30 days of any change.

Notification of changes shall be made in writing and be mailed to the executive
secretary.
(2) Notification of name
changes shall include a copy of a marriage certificate, court decree, or a
social security card reflecting the change and the license replacement
fee.
