Chapter 1370-02 - General Rules Governing Hearing Instrument Specialists
Section 1370-02-.16 - LICENSE

(1) Issuance - Upon the Council determining that an applicant for licensure has successfully met all the requirements as set forth in T.C.A. § 63-17-201, et seq., and these rules, and upon receipt within thirty (30) days of notification in writing of such status, the applicant is required to submit to the Council's office the initial license fee pursuant to Rule 1370-02-.12.

(2) Display of License - Every person engaged in the fitting and dispensing of hearing instruments in this state shall display his license in a conspicuous place in his principal office and, whenever required, exhibit such license to the Council or its authorized representative.

(3) Replacement License - A licensee whose "artistically designed" document or wall license has been lost or destroyed may be issued a new wall license upon receipt of a written request in the Council Administrative Office. Such request shall be accompanied by an affidavit (signed and notarized) stating the facts concerning the loss or destruction of the original license, a recent photograph which has been signed by the licensee on the back, and the required fee pursuant to Rule 1370-02-.06. This process shall be accomplished by the Council's Unit Director.
