Chapter 1350-05 - Rules Related to the Fantasy Sports Act
Section 1350-05-.04 - RENEWAL APPLICATION FOR LICENSURE

Current through April 3, 2024

(1) Any person seeking to renew an application to be a licensed fantasy sports operator shall submit a renewal application, in accordance with T.C.A. § 4-49-208(d) and Rule 1350-05-.05, in a form available from the Sports Wagering Council, no later than forty-five (45) days prior to the expiration of the prior year's license, containing the following information:

(a) All information required in an initial application, as set forth in Rule 1350-05-.03 except for the following:
1. Identity History Summary from the Federal Bureau of Investigation for individuals who have previously submitted criminal background reports as part of the application process and who have not self-disclosed any new criminal history; and

2. Financial statements required by Rule 1350-05-.03(1)(m) 4.

(2) The application shall be signed by an officer or director, member, partner, or individual otherwise authorized by the organization, as applicable in accordance with the fantasy sports operator's corporate structure.

(3) The applicant shall submit a nonrefundable renewal application fee in the form of a check in the amount of three hundred dollars ($300) made payable to the Sports Wagering Council. Review of the applicant's renewal application will not begin until receipt of the application fee.

(4) The renewal application, supporting documentation and fee may be hand delivered or mailed to the Office of the Sports Wagering Council, Attn: Director, at 312 Rosa L. Parks Blvd., 8th Floor, Nashville, Tennessee 37243.
