Section 1320-08-04-.01 - ISSUANCE OF PLATES

The application for a plate must be filed with the owner's or lessee's resident county clerk, and must be supported by a photocopy of form DD-214 or WWK 53-55, or any other similar document issued by the Department of Defense and approved by the Commissioner of Revenue which shows receipt of a Purple Heart by the applicant. The county clerk will issue the plate, and forward a copy of the supporting documentation with the application to the Motor Vehicle Division.
