Section 1255-01-.13 - REPLACEMENT LICENSE OR CERTIFICATE

A license or certificate holder may, by filing the prescribed form with the Commission, obtain a duplicate license or certificate to replace an original license or certificate which has been lost, damaged or destroyed or if the name of the licensee or certificate holder has been lawfully changed.
