A person required to file a statement pursuant to T.C.A. § 56-11-203 shall furnish the required information on Form A, hereby made a part of this Chapter. Such person shall also furnish the required information on Form E, hereby made a part of this Chapter and described in Rule 0780-1-67-.11.
