(1) Fees charged by the Board are as follows:
| (a) | Initial license fee. | $50.00 |
| (b) | License renewal fee. | $50.00 |
| (c) | Late renewal fee. | $10.00 |
| (d) | Replacement license. | $ 5.00 |
