(1) Fees charged by the Board shall be as follows:

Twenty-five dollars ($25.00) |

Renewal of certificate or registration |
One hundred ten dollars ($110.00) biennially |

Initial firm permit |
Fifty dollars ($50.00) |

(e) |
Renewal of firm permit |
Fifty dollars ($50.00) per year |

(f) |
Fee for late filing of permit, certificate, or registration renewal application |
One hundred dollars ($100.00) |

(g) |
Fee for application for reinstatement |
Two hundred and fifty dollars ($250.00) |

(h) |
Transfer of grades or letter of good standing |
Twenty-five dollars ($25.00) per request |

(i) |
Change of address late fee |
