A person may not construct or operate a facility until the person has applied for and obtained a valid permit from the board or secretary. Permits are required before construction begins. Applications shall be made on forms provided by the secretary and shall address the requirements of chapter 74:27:09.
Application forms may be obtained from and completed applications shall be submitted to:
