Section 67:62:13:03 - Requirement for designation and duties of primary provider

A primary provider must be designated for each client in the individualized and mobile program of assertive community treatment program. The designation must be made by the clinical supervisor, be in writing, and be included in the client's file. The designation must be updated as client or personnel needs require. Each individualized and mobile program of assertive community treatment program must have a backup policy to be implemented when a primary provider is not available to serve a client's needs. The primary provider duties include:

(1) Maintaining an orderly and complete clinical file for the client that contains:

(a) Documentation showing that written assessments for the client are completed;

(b) A current case service plan; and

(c) Documentation of services and client responses to treatments; and

(2) Conducting and participating in treatment planning and case conferences with other staff of the individualized and mobile program of assertive community treatment program and with others authorized by the client.
