The department shall provide written notice to the center of the department's intent to suspend or revoke the center's accreditation.

The suspension or revocation is effective fifteen days after receipt of the notice. The notice must contain the reason for the department's action, the opportunity for the center to request reconsideration by the department, and the appeal process.

A center's request for reconsideration must be in writing and received by the department within 15 days of receipt of notification of suspension or revocation.
