A provider must submit to the Department of Social Services, Office of Provider Reimbursement and Audits, a completed copy of the provider's cost report showing the actual costs incurred during the reporting period and the total number of visits for the services furnished as required in chapter 29 of the Independent Rural Health Clinic and Freestanding Federally Qualified Health Center Cost Report Form CMS-222-92, as amended to July 1, 2019. The provider must submit the required cost report to the department within five months after the provider's fiscal year ends.
