The school district must have a care plan for each individual receiving medical services under this chapter. Professionals involved in the child's care must prepare the care plan.
The care plan may not be effective for more than one school year and it must be amended as warranted by changes in the individual's medical condition.
There must be a physician or other licensed practitioners written orders for medical services required under the care plan.
