If an applicant's visual acuity is less than 20/40 in both eyes, with or without correction, the applicant may be restricted to one or more of the following, according to an eye specialist's recommendation and the determination of the department:

(1) Corrective lenses;

(2) No night driving;

(3) No driving outside of city limits;

(4) Left outside rearview mirror; and

(3) [(5)] Not to exceed 50-mile radius of residence.
