If an applicant's visual acuity is less than 20/40 with both eyes, with or without correction, the applicant shall submit a statement of examination to the department, completed by an eye specialist, on a form provided by the department. An eye specialist is an optometrist or a physician skilled in disease and surgery of the eye.
